HMSA has informed us that it will cover Flumist (90660) in the same manner as it did last year, i.e. the eligible will be set at the same amount as the Inactivated Influenza vaccine.  Providers may balance bill the member provided they obtain a signed WAIVER from the member prior to administering Flumist.
